Our Client:

We are partnering with a standout NFP residential, retirement and home care provider who is looking for a Home Care -Clinical Liaison Nurse to complement the team. Situated north of the river and reporting to the Home Care Manager you will be supported in this permanent part time role working 20 – 25 hours per week. There is flexibility with days and hours.

Situated North of the River, you will need to be a Registered Nurse with current registration who has had experience providing support to customers. The role will see you working closely with broker partners to ensure all care and services are monitored, evaluated and achieved. Care will be delivered as per clinical assessments and planning whilst adhering to all clinical governance and home care legislation. You will join a supportive and dynamic team and work alongside the Home Care Manager, Scheduler and Administrator.

The Role:

  • Assessment and coordination of customer services
  • Develop and maintain customers support plans
  • Work with customers and manage the case load
  • Review support plans, risks and adapt to customers’ changing requirements
  • Collaborate with customers, families, support staff and health professionals
  • Provide clinical services with current AHPRA
  • Review and follow up customer clinical issues, identified risks, hazards, incidents
  • Implement and monitor compliance requirement
  • Participate in training support workers
  • Actively propose changes and participate in audits and reviews

The Successful Candidate:

  • Must have current AHPRA registration/clinical background
  • Experience working within Home Care setting
  • Knowledge of Aged Care/Home Care legislation and governance
  • Outstanding customer service focus
  • Experience in continuous quality improvement and risk management
  • Engaging, Passionate and Person-centred approach
  • Current National Police Clearance
  • Current influenza and COVID vaccinations
