Our Client & Opportunity:

Our client is one of Australia’s national high quality, leading Aged Care Providers with various facilities across the whole of Perth and Australia where they provide Low, High and Dementia specific care as well as Extra Services.

This position is situated in the Northern Suburbs of Perth in Edgewater, where you would be looking after a beautiful quaint, small 33 bed facility which is surrounded by beautiful gardens.

The Aged Care Facility is seeking a passionate and committed Care Manager or Lead RN In Charge with Residential Aged Care experience to look after the facility from a clinical perspective whilst the previous Care Manager is on 9 month Maternity Leave.

You will be responsible for leading a team of RNs and Carers who are responsible for delivering high quality care at all times. This position is very ‘hands on’ and you will support the RNs on the floor as and when required.

Responsibilities include:

• Provide high quality, safe and effective nursing care that aligns with individual resident preferences and best practice
• Lead family conferences to resolve issues and complaints
• Identify opportunities for ANACC funding uplift to obtain more services for your residents
• Ensure that all clinical and non-clinical care is delivered in line with the aged care standards
• Coach, mentor and supervise all staff on shift including carers
• Undertake clinical, social and self-care assessments if necessary
• SIRS – management and reporting
• Fall Management and Prevention
• Ensure all care plan documentation is completed with appropriate strategies implemented
• Anticipate residents’ changing health needs and initiate further assessments or referrals
• Complete assessments
• Ensure the facility passes accreditation from a clinical perspective

Skills and experience required:

• Must be able to commit to a 9 month maternity contract
• People leadership experience in a clinical setting
• Highly focused on person centred care practice
• Nursing qualification and current professional AHPRA registration
• Must be experienced in Residential Aged Care
• Understanding of Aged Care Quality Standards and Accreditation is desirable
• NDIS Clearance – be happy to apply for one if you do not already have one
